PCMM first opened our doors in 2013 and have since provided over $1.3 M in free care to our patients.
We seek to BREAK DOWN BARRIERS to receiving quality primary care through a collaborative network of churches and medical providers who work together to provide free services to those most in need and least able to pay for services.

Many in our community have no medical insurance and do not have the financial means to obtain medical care. Our county is also home to migrant farm workers who have a difficult time navigating the current healthcare environment. PCMM seeks to stand in the gap providing primary medical care and disease management.
The patients that we serve do not qualify for Medicaid, are uninsured and have a household income that is less than 200% of the federal poverty guidelines.
